SURVEY INSTRUCTIONS:
Listed on the next page is number of statements that concern the topic of intimate relationships. For the purpose of this questionnaire, an intimate relationship should be thought of as a close relationship with a single partner in which there is some sexual attraction. Please read each of the following statements carefully and decide to what extent it is characteristic of you. Some of the items refer to a specific intimate relationship.
Whenever possible, answer the questions with your current partner in mind. If you are not currently dating anyone, answer the questions with your most recent partner in mind. If you have never had an intimate relationship, answer in terms of what you think your responses would most likely be. Then, for each statement fill in your response by circling the most appropriate number using the scale below on the right hand side of the questionnaire.
The following scale should be used when completing this questionnaire:-
1 = Not at all characteristic of me.
2 = Slightly characteristic of me.
3 = Somewhat characteristic of me.
4 = Moderately characteristic of me.
5 = Very characteristic of me.

Remember to respond to all items, even if you are not completely sure.
